What to Expect
Referral and Initial Connection
Services begin with a referral, after which our team connects with the individual and residential home staff to understand current needs and goals.
Peer Support
Our Peer Support services offer compassionate guidance from individuals with lived experience, helping guests navigate crisis, recovery, and the next steps toward wellness.
Personalized Support Plan
Our multidisciplinary team works with each individual to develop a personalized recovery and wellness plan focused on stability, independence, and successful community living.
Community-Based Services
Support is provided directly in residential homes and the community, offering guidance, care coordination, and education for clients and residential staff.
Ongoing Support and Stability
Our team continues working with individuals and residential providers to strengthen recovery, prevent hospitalization, and support long-term success in the community
No Cost for Services
Our services are funded through local managed care organizations, so there is no charge to guests receiving care.
